Initial Comments:
An off-site follow-up survey conducted on November 6, 2023 found that Primary Providers Home Care, LLC had corrected the deficiencies cited under 28 PA Code, Part IV, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of a re-licensure survey completed on September 12, 2023.
